In order to make payment for your dental services as convenient as possible and maintain the highest standard of comprehensive care, we offer 3 payment options. We will give you and
estimate of your total fees, at the onset of your treatment and will update them as needed. All payment arrangements need to be made prior to the start of your treatment.
Option #1: Total payment of the fee at each visit. Since immediate payment reduces our bookkeeping expenses,
a five (5) percent courtesy reduction of the fee on the service date will be given, as long as the entire account balance is paid in full. The fees may be paid at each visit with cash, check, Visa, MasterCard and Discover. Patients already paying reduced fees are not eligible for reduction. Patients who have insurance benefits are not eligible for reduction. Patients who have insurance benefits are not eligible for discounts due to insurance fraud.
Option #2: CareCredit - As a service to our patients, we are pleased to offer the CareCredit card, the nation's leading patient payment plan. This is a monthly payment plan we offer as a separate dental line of credit to cover you and your family members' dental care needs. Approval only takes a few minutes. There are interest free options as well as low interest extended payment plans. You can make monthly payments as low as 3% of the outstanding balance. There are no annual fees or pre-payment penalties. If CareCredit is your option, you can begin any treatment immediately and spread payments out over time.

Option #3: In house payment plans are available with a minimum of 1/3 of the fee required as an initial down payment, and the balance payable in three (3) monthly installments. With special arrangements through our Patient Account Representatives, longer contract terms may be available; however processing fees and late payment charges apply as with ay consumer credit. For additional convenience, we can
auto-charge each monthly installment on you Visa, MasterCard or Discover card.

If you have insurance benefits, as a courtesy, we will submit a claim on your behalf. A compete patient registration form and/or a copy of the insurance card(s) must be provided by the patient at the first appointment and updated at each additional appointment. Patients must notify our office of any changes with insurance information. It is you responsibility to know your insurance coverage, as you are ultimately responsible for the entire balance. At your request, a pre-treatment estimate form will be submitted to your insurance carrier prior to treatment to obtain an estimate of your insurance benefits. The "estimated" amount not covered by your insurances is payable at the time of service unless other arrangements have ben made.
The cost of dental treatment may be tax deductible and qualify under a cafeteria plan or medical savings account through your place of employment. See your Human Resources representative for details.
